Zahran et al. J Endod 2022;48:479–486 Assessing the Iatrogenic Contribution to Contamination During Root Canal Treatment

Asepsis in endodontics aims to control all potential sources of infection. Inadvertent introduction of bacteria into the root canal system may occur when the aseptic chain is breached during treatment. This study aimed to assess potential bacterial contamination and the potential risk of iatrogenic introduction from 7 sites comprising surfaces, instruments, and files acquired during the treatment of 30 vital teeth with irreversible pulpitis.

Materials & Methods: Bacterial samples were collected from access burs, files, endodontic rulers, rubber dam surfaces, gloves, and instruments. Genomic DNA was extracted and quantified by quantitative polymerase chain reaction. Bacterial types were determined using next generation sequencing.Results: Thirty-eight percent of the initial files introduced into the root canal had significant levels of bacteria at the point of obturation, including endodontic pathogens. Around half of the rubber dam surfaces were contaminated with substantial bacterial loads at the time of obturation, and bacteria were also detected in 20-30% of gloves, instruments and rulers before obturation. Conclusions: Gloves, rubber dams, instruments, and files acquire bacterial contamination during treatment at high frequencies and loads. This highlights the potential risk of iatrogenic contamination at the clinically vulnerable point of canal obturation. Measures to address these may improve clinical outcomes.

I have always been terrified of the idea of a root canal and never thought that I would actually require the procedure. I had a crown seated earlier in the year and knew that it was doomed from the start. When I developed a bump above the gumline over the tooth, I knew it was not going to be an easy fix. Boy, was I wrong. Dr. Nawari quickly diagnosed the issue with a set of x-rays and got straight to work. The process was complete in an hour and was incredibly easy. Dr. Nawari and Dulce were so kind and kept me comfortable and informed throughout. I was given headphones to connect to my phone/Spotify and the procedure was displayed on a screen for me to watch, if I chose to do so. Another set of x-rays concluded the procedure and confirmed that all was looking great, now! I can see why they have such a high rating - they are absolute superstars and so incredibly kind and understanding. Their amazing office administrator, Julie, kindly gave me some thoughtful recommendations for general providers on my way out, as I mentioned to her that I was seeking new general dentistry care. I would not hesitate to come back here for future work, though I hope it's not necessary! I HIGHLY recommend Prime Endodontics and that is coming from someone who is on the upper end of the anxiety scale!

I had the most positive experience here getting my root canal done. When scheduling an appointment, I was surprised how soon it could be scheduled (next day or in a couple of days). Dr. Cohenca is incredibly professional. Before doing the procedure, he performed diagnostic tests in order to be 100% sure that root canal treatment is exactly what I needed. He took great care to inform me of what was going on with my tooth, what he was going to do during the procedure, and answered all my questions. I was also very positively surprised on his technique of administering the local anesthesia. The numbing gel helped with the uncomfortable shot and he took great care to add anesthesia in small bits to make sure I felt no pain. The procedure itself was practically painless and I was provided with chapstick, a blanket, and headphones to put on music from my phone. Very happy with my experience here and 100% recommend Dr. Cohenca.
